Ban Request Format

If you feel like there is a player who has been breaking the rules, and must be punished, post about it here in the following format:

Player's Steam Name: Who was the "violator"?
Player's Steam ID: What was the player's Steam ID? (If possible.)
What happened? A detailed explanation of exactly what happened, clearly pointing out what went wrong.
Who else was a witness to this? Who can we seek out as a reference to this event? Video proof, through YouTube, is a legible proof.
When did this happen? This should include the date, the time and the timezone that this event took place at.

These will be reviewed daily, to make sure we have a fast response time on all inconveniences that may happen. If there is not enough legible proof, or the actions of the "violator" were not grave, the accusations will be dropped and the thread will be locked.
